About the Facility – Baltimore VAMC

The Baltimore VA Medical Center is the acute medical and surgical care facility for the VA Maryland Health Care System and offers a full range of inpatient, outpatient and primary care services. The Baltimore VA Medical Center serves at the neurosurgery referral center for the VA Capitol Health Care Network. The medical center's patient-friendly design features plant-filled atriums, natural light from overhead skylights and comfortable patient rooms and waiting areas. Located next to the University of Maryland Medical Center with a walkway connecting the two facilities, the medical center maintains an active affiliation with the university in the sharing of staff, resources and technology. The primary location is set in Perry Point (a community-based outpatient clinic) and services are needed at the additional location at 10 North Greene St, Baltimore, MD 21201.

Pharmacist - RESPONSIBILITIES (listed are included but not limited to)

Perform a full range of pharmacist procedures.

The Pharmacist will be required to rotate calls with other pharmacists with the infrequent possibility of needing to return to the pharmacy nights, weekends, and holidays for limited services to include, but not limited to, drug information, review and processing of inpatient orders, preparation of IV admixtures, and Pyxis replenishment.

Shall provide outpatient and inpatient pharmacy services to patients.

Will perform work in the following work areas as directed, screening patient electronic health records for appropriateness of the medication order or refill, verifying that patient prescriptions are accurately filled and the directions for use are clear, counseling patients to ensure proper use of the medication.

Order screening duties will include a thorough review of the patient's electronic health record to verify the appropriateness of therapy. The following screening duties list is not exhaustive but rather constitutes the minimum expectation: appropriate dose for the condition, weight, age, and renal/hepatic function of the individual patient, appropriate medication for the condition, monitoring for drug-drug, or drug-disease interactions, and verification that day supply matches what is ordered.

Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.

Counsel patients regarding the appropriate use of medications.

Review patient profiles at the time of dispensing in order to closely monitor medication therapy.

Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancies of the above-listed items.

Compound medications are required.

Centralized Intravenous Admixtures are also required.

Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.

Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain records and security of all scheduled controlled substances according to applicable regulations.

Provide vaccine handling, distribution, and administration support.

Inpatient Pharmacist - REQUIREMENTS

Degree: Must have at minimum a Bachelors Degree in Pharmacy.

Education: Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).

Experience: Two years of professional pharmacy experience after graduation.

Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.

Security: Must possess the ability to pass a Government background check/security clearance.

Life Support Certification: Possess a current AHA or ARC BLS Healthcare Provider Certification.
