Position Overview
In home health and wellness assessments
Adult, Family, and Geriatric Nurse Practitioners and Physician Assistants accepted
Assessments typically last about 45 to 75 minutes
Patient population is geriatric
At least one weekday of availability required
Weekend only availability is not accepted
Travel required within assigned areas
All documentation is completed on an iPad
Clinical Scope
H and P evaluations
Medication review
Vital signs
Finger stick testing
Urine sample collection
Fecal kit distribution
Required Qualifications
Active state license
National certification for NP or PA
CAQH
NPI
Unrestricted license
Auto insurance meeting client requirements
No DEA required
Credentialing/Onboarding Items (must be provided)
Credentialing is self paced and typically 30 to 45 days
Online training modules must be completed before scheduling
Required credentialing documents as requested by the client team
