Job Summary Join our compassionate healthcare team as a Caregiver / CNA / HHA and make a meaningful difference in the lives of those you serve! In this vital role, you will provide personalized patient care, assist with daily living activities, and support individuals across various care settings, including assisted living, nursing homes, and home health environments. Your dedication will help promote comfort, independence, and dignity for clients with diverse needs such as dementia, Alzheimer's, developmental disabilities, and hospice care. Responsibilities
Deliver compassionate assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; accurately document medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.Assist with medication administration following proper protocols to ensure safety and compliance with HIPAA regulations. Support patients with specialized care needs such as catheter care, dementia or Alzheimer's care, hospice support, and memory care routines. Conduct patient observation to identify changes in condition or behavior; communicate findings promptly to supervising healthcare professionals.Prepare nutritious meals and assist with meal planning while maintaining cleanliness through light housekeeping tasks like laundry and cleaning to ensure a safe environment. Maintain thorough medical documentation and ensure compliance with privacy standards; participate in ongoing training related to medical terminology and caregiving best practices.Skills
Proven experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience. Strong knowledge of patient observation techniques, vital signs measurement, medication administration, and medical terminology. Skilled in assisting with ADLs and providing dementia or Alzheimer's care with patience and empathy.Familiarity with EMR/EHR systems for accurate documentation of patient records and medical history. Ability to operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely. Excellent communication skills for working effectively with patients who have developmental disabilities or cognitive impairments.CPR certification preferred; additional certifications like hospice care training or working with individuals with disabilities are a plus.
