Description

Job Responsibilities:
  • Guide users through structured digital care pathways (e.g. Onboarding, First Month Patient Experience)
  • Perform outreach tasks (reminders, check-ins) and follow up on adherence and engagement gaps
  • Conduct one-on-one coaching conversations via telephonic and digital channels (e.g., SMS/messaging, video)
  • Use motivational interviewing and behavior-change techniques to help users set and achieve goals
  • Monitor user engagement, adherence and progress with product (heart sound recordings, daily symptom tracker, weight, BP, other biometrics and behavior logs, etc.) and reach out when slippage occurs
  • Program Design & Optimization
  • Collaborate with operations, product, account management, and other teams to develop and refine coaching program design, content, triggers, and escalation protocols; including the design and refinement of program activities that are completed by the user support vendors
  • Provide feedback on user experience, content gaps, and coaching flows
  • Participate in quality audits, peer reviews, or chart reviews to maintain standards

Skills:
  • Health coaching certification (e.g. NBHWC, Wellcoaches, ACE, CHES)
  • Experience in digital health, telemedicine, or working with health/fitness technologies
  • Training in motivational interviewing or similar counseling methodologies
  • Experience with moderate-to-large caseloads
  • Bilingual / cross-cultural competence
  • Experience partnering with interdisciplinary clinical teams
  • Passion for preventive health, lifestyle medicine, or chronic disease management
  • 8+ years industrial HVAC experience.
  • Strong communication skills—both verbal and written—for remote delivery
  • Empathy, active listening, nonjudgmental approach
  • Familiarity (or willingness to learn) with behavior change theory, motivational interviewing
  • Prior experience in remote coaching, chronic disease management, wellness, or related field
  • Ability to manage multiple DIGITIVA user relationships simultaneously
  • Comfort working in a digital/tech-forward environment (apps, dashboards, communication tools)
  • Commitment to privacy and confidentiality
  • Flexibility and adaptability in a fast-changing environment
  • Prior experience designing chronic disease care management programs

Education/Experience:
  • Bachelor’s degree in a relevant field (e.g., Health, Public Health, Nursing, Nutrition, Kinesiology, Psychology, Health Education, or related)
