Pay Range: $25 - $27/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.

The caseload is 100 members.

This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.

Compliance with care management policies/procedures including outreach and documentation expectations.

Individuals will be audited on a monthly basis to ensure charts are in compliance.
