Community Health Worker (CCHW) - Remote (Florida Only)
Pay: $23/hour | Full-Time
Must reside in Florida
We're hiring an empathetic, self-directed Certified Community Health Worker (CCHW) to support Medicare patients in a fast-paced, growing program.
This role is 90% remote with 2-3 local site visits per month (expenses reimbursed).
Requirements
Active CCHW certification (required)
2+ years of community health, patient advocacy, or case management experience
Ethernet (hard-wired) internet connection required
Quiet, private home workspace (no public spaces)
Strong time management and attention to detail
What You'll Do
Provide patient advocacy and resource coordination
Connect members to community services
Conduct occasional local outreach visits
