Role Description
This is a contract hybrid role as an Internal Medicine Physician specializing in Post-Acute Care/Skilled Nursing Facilities (SNF), located in Long Beach, CA, with the flexibility for some work-from-home charting.
Responsibilities include conducting comprehensive medical evaluations, developing and managing patient treatment plans, collaborating with interdisciplinary healthcare teams, and monitoring patient progress.
The role will also require oversight and coordination of patient transitions of care to ensure continuity and quality.
Supervising an APP is an option with this role.
We have full back-office support with additional scribe support services.
Qualifications
Strong clinical skills in Internal Medicine and experience in post-acute and skilled nursing facility care
Effective communication and collaboration skills with patients, families, and interdisciplinary teams
Familiarity with electronic medical records (EMR) systems and ability to maintain accurate and timely documentation
Capability to work autonomously and make informed decisions in a variety of healthcare settings
Current unrestricted medical license in the state of California
Board certification or eligibility in Internal Medicine
Commitment to compassionate, patient-centered care and enhancing patient outcomes
Experience in quality improvement initiatives or care transitions is a plus
Set your own schedule/hours.
This is designed to be a part-time supplemental opportunity but can transition into full-time work.
Compensation is volume based.
Compensation from $250,000-$350,000 annually.
