Description
Our client is seeking a Care Management Services Director to lead the strategic development, planning, and management of care facilitation services. This role oversees case management, social work, utilization management, and post-acute service coordination to ensure efficient patient movement through the hospital continuum. The ideal candidate will drive program development, enhance staff roles related to patient progression, and foster collaboration with physicians and healthcare teams.

Responsibilities:Oversee and develop hospital case management programs.

Manage case management, social work, utilization management, and post-acute service coordination.

Enhance care progression by defining staff roles and ensuring collaboration with physicians.

Implement and refine processes to improve patient flow and care efficiency.

Maintain compliance with healthcare regulations and best practices.

Must Haves:Licensure – Current state RN License.

Experience – Five years of case management experience in a healthcare setting.

Skills – Strong communication and customer relations abilities.

Nice to Haves:Master’s degree preferred.

Three to five years in a supervisory role preferred.

Formal case management training preferred.

Location: Missoula, MT

Salary:  $85,500 – $128,000 Signing Bonus: Available
