Virtus Healthcare Staffing is seeking a dedicated and compassionate Physical Therapist (PT) to join our partner, a leading healthcare facility in Modesto, California. In this role, you will provide essential physical therapy services to participants in both center-based and home-based settings, helping them achieve their highest level of physical function, well-being, and independence.

Position Overview:

As a Physical Therapist, you will conduct comprehensive assessments, develop individualized care plans, provide evidence-based therapeutic interventions, and collaborate closely with a dedicated interdisciplinary team (IDT) to enhance the lives and functional abilities of older adults participating in the program.

Responsibilities:

Assessment and Care Planning:

Conduct comprehensive physical therapy assessments in both the facility and participants' homes to accurately identify their functional needs, preferences, and rehabilitation goals.

Collaborate effectively with the interdisciplinary team (IDT), including the Primary Care Physician, rehabilitation team colleagues, and other members, to develop holistic and individualized care plans that promote participant independence and safety.

Therapeutic Interventions:

Deliver skilled, evidence-based physical therapy interventions in accordance with physician orders and the established care plan.

Provide a diverse range of treatments tailored to geriatric patients, including active/passive exercises, muscle re-education, gait and balance training, transfer training, functional training for activities of daily living (ADLs), and appropriate modalities (e.g., massage, heat, ultrasound, electrical stimulation).

Supervision and Training:

Supervise and mentor Therapy Assistants, providing clear guidance and training on the correct implementation of Functional Maintenance Programs (FMPs).

Educate clinical staff, participants, and caregivers/families on the proper use, care, and maintenance of therapeutic equipment (e.g., mobility devices, adaptive equipment).

Collaboration and Communication:

Communicate effectively and regularly with participants, families, caregivers, and the IDT to ensure coordinated, patient-centered care and smooth transitions.

Maintain accurate, thorough, and timely documentation of all assessments, interventions, participant progress, and communication in the medical record.

Qualifications:

Education: Bachelor's Degree in Physical Therapy (Required); Master's Degree in Physical Therapy or Doctorate in Physical Therapy (DPT) (Preferred). (Note: If degree was earned outside the US, validation of US equivalency is required) .

Experience: Minimum five (5) years of experience as a licensed Physical Therapist (Required); Minimum two (2) years of experience working clinically with a frail or elderly population (Preferred).

License: Valid California Physical Therapy License (or proof of license eligibility) (Required).

Certification: Current CPR certification (Required).

Skills: Strong clinical assessment and treatment skills in geriatric physical therapy; Comprehensive knowledge of various treatment modalities and therapeutic exercises appropriate for older adults; Excellent communication (verbal/written), interpersonal, and teamwork skills; Demonstrated ability to work effectively both independently and as part of an interdisciplinary team.
