General Summary:
Under the direction of the Supervisor, Rehabilitation, provides Physical therapy services to patients following the plan of care established by a Physical Therapist.
Education, Licensure, Registration and Experience:
Graduate of an approved associate degree program as a LPTA
Previous Clinical experience/rehabilitation experience preferred
State Licensure as a LPTA
CPR Certification
Essential Functions:
Implements the plan of care developed by the Physical Therapist.
Instructs client and caregiver in treatment programs, precautions, and equipment use as necessary and documents client/caregiver demonstration of instructions provided.
Documents client status and progression towards achievement of outcomes on each visit in measurable parameters.
Submits accurate and complete billing for each patient.
Is responsible to work within the guidelines and authorizations provided by the individual's insurance coverage, AND/OR Medicare guidelines.
Demonstrates dependability/flexibility in meeting scheduling needs of the department and facility.
Consistently strives to be a productive member of the institution and always puts time to good use.
Consistently progresses patient and adds to home ex. program.

Standards of Employment:
Attends all mandatory education programs and can describe his or her responsibilities related to general safety and regulatory compliance.
Appearance is professional, neat, clean, and appropriate for the work environment. Wears proper photo identification badge while on duty.
Maintains confidentiality of all hospital and patient information at all times, as observed by peers and management.
Attends monthly staff meetings and participates in monthly rounding process with manager to assure effective and timely two-way communications.
Participates in and completes required department specific education and training programs and can describe his or her responsibilities related to department safety and specific job-related hazards.
Follows safety and security policies including exposure control plans/blood borne and airborne pathogens.
