Registered Nurse, Chino

This entry‑level position does not supervise other registered nurses and is responsible for conducting and completing initial, comprehensive, and updated patient assessments while providing hospice services requiring nursing skills in accordance with the plan of care and the Nursing Practice Act. The role assists patients and families in identifying goals and areas of need, contributes to the development of treatment plans, and regularly evaluates their effectiveness. It includes assessing patient status and developing, implementing, and evaluating the Individualized Plan of Care in collaboration with the Interdisciplinary Group to ensure continuity of care. Duties also involve monitoring, assessing, and documenting patient progress, completing all clinical notes in a timely manner, and coordinating care with the Interdisciplinary Group. The nurse participates in QAPI and Infection Control processes, submits clinical record audits promptly, and provides patient and family education while maintaining a safe environment. Additional responsibilities include supporting continuity of care across settings such as homes, skilled nursing facilities, residential care facilities for the elderly, and board‑and‑care facilities; providing ancillary services and supervision when needed; participating in on‑call duties; managing schedules to attend required meetings; and safeguarding all patient information in compliance with HIPAA regulations. Hospice care is provided wherever the patient is located within Orange, Los Angeles, Riverside, and San Bernardino Counties.
