Dietitian, New York

Managed Care Organization is seeking a Full Time Dietitian to join their team!

Location: Flushing
Schedule: Monday – Friday 9am-5pm (FT)

DUTIES INCLUDE:

Conducts face-to-face and telehealth nutrition assessments and reassessments using professional guidelines.

Evaluates physical conditions and creates individualized therapeutic diet plans.

Provides clinical nutrition recommendations, subject to physician or nurse practitioner approval.

Maintains updated participant files with diet orders, restrictions, preferences, and assessments.

Develops and monitors individualized nutrition care plans and adjusts as needed.

Documents care and changes in participant condition per agency standards.

Coordinates care with interdisciplinary teams and revises plans based on progress.

Offers nutrition counseling and education to participants and caregivers.

Participates in team meetings, quality reviews, and professional development.

Conducts home visits, evaluates meal and supplement needs, and engages in nutrition-related programs and staff training.

QUALIFICATIONS:

Bachelor's Degree from an accredited Dietetics program.

- A minimum of one (1) year of experience in clinical nutrition.

- Minimum of one (1) year of experience working with a frail or elderly

population or, if the individual has less than one (1) year of experience but

meets all other requirements, must receive appropriate training from the

PACE organization on working with a frail or elderly population upon

hiring.

- Home Care experience, preferred.

- Current registration to practice as a Registered Dietitian through the

Commission on Dietetic Registration – Registered Dietitian Nutritionist

(RDN).

- New York State license: Certified Dietitian Nutritionist (CDN).
