Description JOB SCOPE: The Client Care Coordinator is responsible for improving access to mental health services for families experiencing homelessness in the Family with Children shelter system. This role supports families in navigating complex systems and managing the emotional and psychological stressors associated with housing instability. Through this role, DHS aims to: Enhance the delivery and coordination of mental health and related services to families in shelter. Promote best practices among shelter social services staff. Improve linkages to mental health and community-based resources. Increase cultural and linguistic competence in addressing mental health. Strengthen permanency outcomes using a family-driven and youth-guided approach. Duties and Responsibilities Complete comprehensive biopsychosocial assessments for each family. Provide short-term counseling, crisis intervention, risk assessment, safety planning, and psychoeducation. Collaborate with ACS and preventive service agencies on child welfare concerns. Participate in ACS conferences and advocate on behalf of families. Make appropriate referrals and link families to needed resources. Facilitate group work (e.g., with youth or fathers). Communicate and collaborate with external service providers to ensure effective engagement. Educate and train shelter staff on mental health and trauma-informed care. Facilitate quarterly family meetings with shelter staff and families to discuss planning and progress. Document all client services, assessments, and interventions in accordance with program protocols. Attend team meetings and case conferences. Perform other duties as assigned by the supervisor or program director. Qualifications Master’s degree in social work from an accredited institution. LMSW required or eligible within 6 months of hire. Strong knowledge of child/adolescent development, emotional/behavioral health, and parent-child relationships. Familiarity with diagnostic classification systems and family dynamics. Experience in strengths-based, solution-focused, and family-centered practice. Understanding of child welfare regulations, permanency planning, and trauma-informed care. Experience working with diverse cultures and populations. Excellent verbal and written communication skills. Bilingual proficiency (English/Spanish) preferred. Proficiency with Microsoft Office and electronic case management systems.
