Compensation & Perks
Base Salary: $130K+ (flexible based on experience)

Monday-Friday, 8:00 AM - 5:00 PM

As a Nurse Practitioner, you'll deliver comprehensive primary care in a collaborative, team-based setting.
You'll manage chronic conditions, guide patients through their care journey, and build meaningful long-term relationships.
Responsibilities include:
Patient assessments, exams & diagnostics
Chronic disease management (diabetes, COPD, CHF, dementia, etc.)
Developing personalized care plans
Patient & family education
Coordinating care with interdisciplinary team
Clinic-based care with some home visits & telehealth
What We're Looking For
Board-Certified NP (Adult, Adult-Gero, or Family)
Active NP license & DEA (or eligibility)
2+ years of outpatient primary care experience required
Comfortable working independently
Experience with value-based care (preferred)
Athena EMR
chronic disease management, wound care, injections, ABIs, or imaging interpretation
