MDS Coordinator, Port Royal

Beaufort Nursing and Rehab - Wonderful facility looking for a full-time MDS Coordinator to work alongside our great nursing team. Pleasant work environment and excellent compensation. RN preferred. MDS Coordinator Scheduling and completing all MDS assessments in a compliant and efficient manner as described in the RAI Manual. Maintaining a monthly OBRA MDS list along with the care planning schedule and care plan reviews. Assisting the team with the care-plan process and ensuring all triggered items are care planned. Educating the IDCPT on the MDS and RAI process as appropriate. Completing Medicare-required MDS assessments and staying up to date with PDPM coding and reimbursement policies. Coordinate and integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure substantiated reimbursement. Completing the Physician Medicare Certification and Recertification Coordinating the weekly“UR” meeting Performing the monthly tiple check process Assisting the team with ICD-10 code selection and diagnosis input into PCC. Attending pertinent meetings as needed. IND123
