Role Description

Life Choices Center is seeking an experienced Registered Dietitian Nutritionist (RDN) or Certified Dietitian Nutritionist (CDN) to lead and help build our Healthy Choices Program —providing nutrition counseling, education, and food prescription services to individuals and families navigating complex and often vulnerable circumstances.
This is a unique opportunity to be instrumental in shaping and growing a new program .
You'll combine evidence-based nutrition care with a mission-driven, faith-informed approach , helping clients access both the resources and the support they need to move forward with confidence.
What You'll Lead Individualized nutrition counseling and care planning
Nutrition assessments and ongoing client support
Food prescription services through the NYS Medicaid Waiver **** Program
Nutrition education (1:1 and group settings)
Collaboration with a multidisciplinary, community-focused team
Development and expansion of the Healthy Choices Program
