MTM Pharmacist
Redefine patient engagement through personalized, remote MTM services.
Key Responsibilities: Conduct telephonic or video MTM sessions for Medicare and commercial patients.
Document clinical interventions and CMRs in compliance with CMS standards.
Identify and resolve medication-related problems and adherence barriers.
Qualifications: PharmD and MTM certification preferred.
Prior MTM, chronic disease management, or ambulatory care experience.
Ability to connect and build trust with patients virtually.
