Job Description:

The Office of Mental Health (OMH) is seeking a qualified Social Worker to provide mental health services to forensic patients at the Five Points Mental Health Unit. This role involves direct therapeutic care and clinical documentation as part of a multidisciplinary team.

Job Duties:

Serve as a primary therapist, delivering individual therapy to assigned patients.

Conduct and document mental health assessments, including:

Core Histories

Suicide Risk Assessments

Weekly/Monthly Progress Notes

Treatment and Discharge Plans

Facilitate group therapy sessions as needed.

Maintain accurate and timely clinical documentation in accordance with OMH policy and procedure.

Collaborate with other clinical and correctional staff to support patient care.

Minimum Education and License Requirements:

Must possess a valid New York State license and current registration as a Licensed.

Master Social Worker (LMSW).

Preferred Skills:

Experience working in forensic or correctional mental health settings.

Strong clinical assessment and documentation skills.

Ability to work independently and collaboratively in a structured environment.

Miscellaneous Information:

Approximately 40 hours per week (Monday Friday).

Exact shift/hours to be discussed during the interview.

Candidate must submit proof of current TB testing, and a recent flu shot.

May have contact with incarcerated individuals.

Completion of Central New York Psychiatric Center (CNYPC) orientation is required prior to starting.

Interviews will be conducted in person or online based on the availability.

Training will be provided.
