PURPOSE:
This scope of work is for Web Applications Programming services for technical support to the Child
Protection Team Information System (CPTIS) and Information System for the Sexual Abuse Treatment
Program (ISSATP), which was built in a Microsoft ASP.NET/SQL Server environment for the Division of
Children's Medical Services, Bureau of Child Protection and Special Technologies (CPT). Contractor will
provide these services to the Florida Department of Health (Department).
2. TERM:
This scope of work will begin on 7/1/2025 or the date on which the purchase order is issued, whichever is
later. It will end at midnight, Eastern Time on 6/30/2027. The State of Florida's performance and obligation
to pay under this purchase order and any subsequent renewal is contingent upon annual appropriation by
the Legislature and satisfactory performance of the Contractor.

4. CRIMINAL BACKGROUND SCREENING:
The Department will conduct a criminal history record check, including fingerprinting, on the consultant
assigned by the Contractor prior to the consultant commencing work for the Department. The Department
retains sole discretion as to whether the results of the criminal history record check will result in the
Contractor's employee being disqualified from performing services pursuant to this agreement.
5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE:
Contractor staff assigned to this agreement must possess the following minimum qualifications and
experience:
5.1. A minimum of 6 years of direct experience developing web-based applications in a Microsoft ASP.Net
Environment.
5.2. A minimum of 6 years of direct experience with object-oriented development techniques.
5.3. A minimum of 6 years of experience with Microsoft SQL Server 2005 or higher.
5.4. A minimum of 6 years of experience/knowledge of SQL programing.
5.5. A minimum of 6 years of experience with Crystal Reports 11.00 or higher.
5.6. A minimum of 6 years of experience/knowledge producing work products using Department of Health
Office of Information Technology required languages and formats.
6. CONTRACTOR RESPONSIBILITIES:
6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified:
6.1.1. Provide Web Application Programming support, ongoing maintenance, and required
modifications to the Child Protection Team Information System (CPTIS) and Sexual Abuse
Treatment Information System (ISSATP), as directed by the Department.
6.1.2. Provide the Department with an additional Contract Web Applications Developer, as requested,
in the event the assigned Contract Web Application Programming Developer has a planned or
unplanned absence longer than one business day.
6.1.3. Provide research, analytical services, and trainings to the Department's Division of Children's
Medical Services, Bureau of Child Protection and Special Technology as directed by the
Department.
6.1.4. Meet weekly with the Department program office to collect user requirements for the
enhancements to the existing ISSATP system, as directed by the Department.
6.1.5. Develop and submit screen layouts for the enhancements to existing systems to the
Department's Division of Children's Medical Services, Bureau of Child Protection and Special
Technology as needed and as directed by the Department.
6.1.6. Develop and submit system report layout for the enhancements to existing systems to the
Department's Division of Children's Medical Services, Bureau of Child Protection and Special
Technology as needed and as directed by the Department.
6.1.7. Develop and submit work estimates for system enhancements or new development to the
Department's Division of Children's Medical Services, Bureau of Child Protection and Special
Technology as needed and as directed by the Department. Provide work estimates upon request
in writing via e-mail to the Department's Application Administrator. No work is implemented
without receiving prior written approval from the Department. Store the work estimates with
project archives or as directed by the Department.

6.1.8. Make performance tuning system recommendations as directed by the Department.

a web application. Ensure the developer updates database indexes or identifies

6.1.9. Perform testing on the system as directed by the Department. Testing includes unit, system, and
integration testing and results will be provided in writing via e-mail to the Department's
Application Administrator and stored with project archives. No work is implemented without
receiving prior written approval from the Department.
6.1.10. Participate in Change Management Meetings as requested with Department's program office
information systems development manager.
Develop and submit documentation related to the Department's CPTIS Applications
Administrator as requested by the Department's Special Technologies Unit Director, the Child
Protection and Special Technologies Bureau Chief or as directed by the Department. This
documentation will be in the form of:

6.1.12. Provide the Department's Division of Children's Medical Services management with systems
status as directed by the Department.
6.1.13. Resolve any issues and problems identified by AppScan as needed and as directed by the
Department.
6.1.14. Maintain accurate and detailed accounting of hours worked daily and submit to the Department's
Applications Administrator as directed by the Department.
6.1.15. Ensure the Contractor's Developer follows Department information technology procedures and
coordinate with other Department information technology staff to ensure the continued
compliance to Department standards as directed by the Department.
6.1.16. DATA SECURITY AND CONFIDENTIALITY TASK:
The Contractor, its employees, subcontractors, and agents must comply at all times with all
Department data security procedures and policies in the performance of this scope of work as
specified in the Data Security and Confidentiality document attached to the purchase order.
6.2. DELIVERABLES:
Contractor will complete and submit the following deliverables to the Department in the time and
manner specified:
6.2.1. Monthly: Provision of Web Applications Programming services in the time and manner specified
in Tasks 6.1.1. though 6.1.16.
7. METHOD OF PAYMENT:
7.1. A purchase order will be issued to the Contractor.
7.2. The method of payment for this purchase order is unit rate.
7.3. The Contractor will not receive payment in advance for goods or services described in this scope of
work.
7.4. The Contractor must submit an invoice monthly that provides a detailed accounting of the deliverables
performed during the invoice period for which payment is being requested.
7.5. The Contractor is responsible for the performance of all tasks and deliverables contained in this scope
of work.
8. PERFORMANCE MEASURES AND FINANCIAL CONSEQUENCES:
All deliverables and related tasks must be completed 100% as specified. Failure to satisfactorily complete or
submit a deliverable in the time and manner specified will result in a financial consequence as indicated
below:
8.1. Failure to complete and submit Deliverable 6.2.1 in the time and manner specified will result in a
payment reduction equal to 60 percent of the total monthly invoiced amount.
8.2. Failure to comply with Task 6.1.16, DATA SECURITY AND CONFIDENTIALITY will result in a payment
reduction equal to 15 percent of the total monthly invoiced amount for each month the Contractor is
not in compliance.
