We are seeking an experienced APRN with cardiology expertise to provide high-quality, patient-centered care in rehabilitation hospitals and SNFs.
This in-person role involves managing complex cardiac conditions and requires availability 3 5 days per week, including weekends and occasional after-hours on-call coverage.
Responsibilities:
Deliver direct cardiovascular care in skilled nursing, rehabilitation, and long-term care settings
Perform focused cardiovascular assessments and comprehensive physical examinations
Evaluate, diagnose, and manage acute and chronic cardiac conditions (e.g., heart failure, CAD, arrhythmias, hypertension)
Develop and implement individualized cardiac treatment and monitoring plans
Prescribe and manage cardiovascular medications in accordance with Florida guidelines and evidence-based cardiology standards
Interpret and trend cardiac-related diagnostics (EKGs, labs, vitals, weights, fluid status, symptoms)
Complete timely and accurate documentation in the EMR/EHR
Conduct cardiac-focused admission, follow-up, and discharge evaluations
Coordinate care with cardiologists, attending physicians, nursing staff, and ancillary services
Promote cardiovascular risk-factor modification and preventive care
Ensure continuity and quality of cardiac care with prompt interventions to prevent decompensation
Participate in interdisciplinary rounds and initiatives to reduce cardiac-related hospital readmissions
Educate patients and families on cardiac conditions, medications, lifestyle changes, and warning signs
Comply with infection control standards, regulatory requirements, and facility policies
Required Qualifications:
Active Florida APRN license
Active NPI number
Minimum of 1 year of cardiology clinical experience
Minimum of 3 years of nursing experience (skilled nursing or post-acute care preferred)
Current BLS and ACLS certification
Strong clinical assessment, communication, and independent decision-making skills
Experience with EMR/EHR systems (PointClickCare and/or Matrix preferred)
Ability to work independently in a fast-paced clinical environment
Knowledge of post-acute care and geriatric best practices
Working knowledge of state and federal healthcare regulations, HIPAA, and patient safety standards
Excellent written and verbal communication skills for documentation and interdisciplinary collaboration
Compassionate, patient-centered approach to care
DEA registration (preferred)
