Inpatient Case Management Coordinator
The Inpatient Case Management Coordinator plays a critical role in supporting case managers and social workers by coordinating care, managing documentation, and ensuring timely communication with members, providers, outside facilities and internal teams.
The Case Management Coordinator will work closely with case managers, social workers, providers and interdisciplinary teams to facilitate care coordination.
This position requires strong organizational skills, attention to detail, and an understanding of managed care processes to facilitate seamless transitions of care and optimize patient outcomes.
The Case Management Coordinator assists with the management of members, Out of Network and SNF facilities.
Responsibilities include safeguarding the confidentiality of patient's protected health information, ensuring a safe patient environment, updating daily census reports, communicating with members, collaborating with case managers and social workers, verifying member eligibility and benefits, maintaining understanding of managed care principles, scheduling member appointments, conducting follow-up calls with discharged patients, entering and updating all inpatient admission authorizations, assisting case manager with creating authorizations required for safe discharge, facilitating delivery and tracking of required forms, preparing and distributing daily reports for patient rounds, receiving, reviewing, labeling, and filing incoming faxes, ensuring complete and accurate documentation, assisting in completing of health plan reports, fostering positive relationships with physicians, facility staff, and colleagues, adhering to Enterprise policies and procedures, engaging in professional development, assisting in training new hires, providing coverage for team members, and performing other duties as assigned.
Teamwork/Customer Service Responsibilities include treating each person with respect, displaying loyalty and pride, upholding confidentiality, demonstrating commitment to open communication, and maintaining a clean and safe environment.
Personal Qualities include the ability to recognize differing values, make good decisions, set priorities, follow through on assigned tasks, maintain strict confidentiality, and interact in a professional, courteous, and patient focused manner.
Skillsets include high level of communication skills, excellent organizational skills, strong attention to detail, proficiency in Windows, Microsoft Word, and Excel, minimum typing speed of 40 wpm, and knowledge of medical terminology.
Education required includes a high school diploma or GED and minimum of two years of experience in health care field.
Preferred experience in case management, managed care, or healthcare coordination and Spanish speaking.
Shift Hours: 8:30 AM 5:00 PM.
